I think this is one of the toughest questions to answer.
There is no perfect answer to this question, unless one is really OK. (or at least in my case)
If I'm ok and you ask me this question, I will say im ok.
If I'm not ok and you ask me this question, I will still say im ok.
If I'm not ok and I say that I'm not ok, the ball is thrown back to your court and you're then required to respond. And that might not be something you're totally comfortable with (or even worse, expecting).
When you do not respond, or come up with a cant-be-bothered response, it will only serve to worsen the degree of non-okayness.
So isn't it better if the answer is always OK?

most people aren't genuinely concerned.
just for the sake of starting a conversation/breaking the silence.
3 simple words (in each case) that are so commonly misused -in my opinion.

harmless ONLY when the other party is looking for a mindless conversation too i suppose. hahha
BUT, i must say, 'are you ok' is much more sincere.
id think its when people really want to hear you.
so, in this case, better to always say OK? -- NO.

p/s : it also depends on who its coming from.
was asked "r u ok with it?" for a decision that has been made at a point of no return.
how did i feel? cynical.
and the last time i asked someone who didnt look too good "u ok?" just to let the person there's someone who cares. nothing much i can help, but this.

Okay now that I look at it from another perspective, I do agree that "are you ok?" is essentially an expression of concern. If anything, it can only be positive.
That said, I still don't know if my answer will always be OK. "人和" 还是最重要的吧。
